Embed Microsoft Forms into Your Website

1: Follow these steps on How to Embed Microsoft Forms into Your Website Page. Log in to your Microsoft account and access Microsoft Form

2. Click Collect Responses at the top-right corner of the form

3. Click Embed and Copy the Javascript code

4. Go to the web page you want to insert the code

My website page in this instance is my index.html and I will insert the embedded code inside the body.

Once the code is properly inserted into the right location inside your web page code then you will be able to view the form directly from your website. Your website visitor can send information to you via the form.
